Claiming US gambling winnings tax refund

Hello.. I won $7000.00 US in Las Vegas and 30% of the winnings were withheld. How do I go about claiming the taxes? Are there any forms ? I am a Canadian Citizen.. Thank you for your advise and help.

No problem.
You need a US ITIN which you get by filling in US form W7.  You can find the W7 at: 
http://www.irs.gov/pub/irs-fill/fw7.pdf
Then we need some sort of record of ALL the money you won (not just the $7,000) and all the money you lost.
If you lost more than your won, there is a refund.  If you won more than you lost, the US is entitled to their 30% of the winnings.
Right now, the IRS has $2,100 US.  
If it turned out that you had actually won $3,000 Net over the year, you would have to leave 30% or $3,000 or $900.00 behind.
