US citizen with Canadian RRSP wants to know about forms

I have a Canadian RRSP that I contributed to this year.
Do I have to send in both 3520 and 3520A or just 3520?
Are there any 'example' 3520's on the web that I could use to
help me fill out the form?

Neither, unless you took money OUT of the RRSP.
You can find the instructions on our website at www.centa.com,
click on newsletters and go to August or more simply just click
here:
 http://www.centa.com/august.htm
If you wish to retain us to look after them, the charge would be
$300 Canadian for the first RRSP and $100 for each additional
RRSP.
We would need a copy of your 2002 US 1040 and the Dec 31st, 2001
and Dec 31, 2002 year end statements for each RRSP you have.
After we prepare them this year, you should be able to copy the
method in the future.
I am assuming that you have also filed your TDF forms for all
your financial accounts in Canada.  Failure to file these forms
can result in a fine of up to $500,000 plus five years in jail
and they are paying extreme attention to these
