My husband worked in the US for 4 years with an H1B visa, I never worked and never had a work visa and he left the job in the US in May last year and we moved back to Ontario where he got a job in August.  We intend to stay in Canada permanently now.  Do we have to file in both countries and would we declare both sources of income in each?

You will file a Dual Status US 1040 or a 1040NR or a Full year 1040 by reporting your Canadian Income on the US return and claiming foreign tax credits for the tax paid to Canada.  This last suggestion will likely get  you the most refund in the US because you get full standard deductions AND you can file a Joint return which you can NOT do if you file a dual status 1040 or a 1040NR.
You will also file a US non-resident or part year resident return for the State you lived in unless it was one of the seven states with no tax form such as Nevada, Washington, Florida, Texas, Alaska, etc.
You will file an Arriving resident return for Canada and Ontario.  So, if you lived in Texas or Florida and moved back to Ontario, you will file two returns (Canada and Fed US). If you moved from Michigan to Ontario, you will file a Michigan, Canadian and US Federal return.
However, if you had moved from Michigan to New York city, you would be filing a Michigan, New York city, Now York State and Federal Return.  And if you moved from New York City to Montreal, you would file a New York City, New York State, US Federal, Canadian Federal and Quebec Provincial return.
